The last warning letter from the U.S. Food and Drug Administration (FDA) for 2015 apparently went to Eco Sushi Express Inc. in Oakville, CT. 
In the letter sent on Dec. 29, FDA said it found “serious violations” of the seafood Hazard Analysis and Critical Control Point (HACCP) regulations during inspections
